No. Text recognition (OCR) is done entirely on your device with Apple Vision, and your photos and lines are never sent to a server. For details, see the Privacy Policy.
Text recognition is done by iOS on your device. It reads books in Korean, English, Japanese, Chinese, Russian and most Western European languages, and on recent versions of iOS also Thai, Vietnamese, Turkish, Indonesian, Polish, Czech, Dutch and the Nordic languages. Greek text cannot be recognized yet.
If you are signed in to iCloud with the same Apple Account and iCloud Drive is turned on, your saved books and lines sync between your devices automatically.
Cover recognition guesses from the size of the lettering, so it can get it wrong on very decorative covers. You can correct the title and author in the form before saving the book.
After photographing the page, rub a finger over the sentence you want, as if using a highlighter, and only those words are selected. There is also an eraser mode, plus Select All and Clear All.
Check in Settings › Bookmark Shot that access to the camera and photos is allowed.
